ACTAge aims to overcome the fragmentation of services for older adults and promote a care model that focuses on rights, autonomy, and well-being, in accordance with the Dutch Positive Health approach, which considers health in all its dimensions—physical, emotional, and social.

The partnership involves institutions and local governments from ten European countries—including Italy, represented by Finpiemonte—and brings together municipal authorities, regional, and national organizations in an international collaboration network.

Among the project's distinctive features is an in-depth exploration of the Social Outcome Contracting model, which links funding to concrete and measurable results, promoting resource efficiency, service quality, and sustainability.

As a partner, Finpiemonte, in synergy with the relevant regional directorates, will contribute to the exchange of experiences and policy innovation, strengthening its ability to propose innovative solutions and new financing models.

ACTAge thus represents an opportunity to renew policies for seniors, reduce the costs of acute care, promote inclusive communities, and concretely improve the quality of life of older people in Piedmont and Europe.
